What Makes Everhaven Hospice Different?

Everhaven Hospice was founded by a group of seasoned hospice professionals to create a safe haven for patients, families, employees, and community partners. Each member of the Everhaven team shares a desire to serve with compassion, integrity, and genuine care.

We strive to be a valued resource in the community to individuals, families, and caregivers facing the unique physical, emotional, and spiritual needs that arise at end-of-life.

We don't believe end-of-life care should feel institutional or impersonal. Our interdisciplinary team — nurses, physicians, social workers, chaplains, and aides — wraps around each patient and family with coordinated, compassionate support that honors your values, your wishes, and your dignity.

What Is Hospice Care?

Hospice is a type of compassionate, comfort-focused care for individuals in the final stages of a serious or terminal illness — when a medical professional identifies that a person's life expectancy is approximately six months or less and curative treatment is no longer effective or desired. The goal of hospice is to enhance quality of life, relieve pain, and support the emotional, spiritual, and practical needs of both the patient and their loved ones.

The Everhaven Hospice Team

Whole-person care at every level. Here is what our interdisciplinary team includes.

Hospice Medical Director

Coordinates care with the hospice team to ensure pain and symptoms are properly managed. In most cases, the Hospice MD will act as the patient's attending physician once admitted to hospice.

Registered Nurse Case Manager

Specializes in pain and symptom management to ensure patients have proper medications, medical equipment and supplies through routine visits and open communication. Coordinates care with the full team, family, and community partners.

Certified Nursing Assistant

Assists with bathing, dressing, and other basic hygiene needs. CNAs also provide basic education to caregivers in an effort to prioritize the patient's safety and dignity.

Licensed Social Worker

Advocates for a patient's end-of-life values and desires, assists with community resources, and acts as a source of supportive counsel for the patient and family while helping to navigate difficult emotions.

Chaplains

Provides spiritual and emotional support to patients and families, regardless of religious beliefs or spiritual backgrounds. Chaplains provide companionship and support to bring comfort, peace, and meaning during the end-of-life journey.

Bereavement & Volunteers

Our Bereavement Coordinator provides grief support to the family and loved ones after a patient passes. Hospice Volunteers provide non-medical, compassionate support through companionship visits and practical assistance.
